#1. Sentinel Events and Root Cause Analysis: Is the "voluntary self-reporting" that JCAHO talks about really voluntary?
Yes. Speak with your JCAHO liaison
person and your facility counsel, but as of this writing, our
interpretation of the JCAHO Standards, Intents and policies is that
should the JCAHO become aware of a sentinel event at your facility,
you must be able to demonstrate that you have conducted a
"thorough and credible" root cause analysis within
the time limits specified by whatever JCAHO policy is then applicable
(currently within 45 days of the discovery of the event by your
facility). The reporting is voluntary, the analysis is mandated.
Note, however, that if your
hospital is a Department of Defense healthcare facility, you may be
required both to report your sentinel events and send your root cause
analyses to the JCAHO. This is also true for certain states.
