#7. You seem to frequently use the term "adverse event or condition" rather than "sentinel event." Why?We use the term "sentinel event" primarily in discussions of JCAHO issues, for the sake of conversational and instructional clarity. However, we feel that root cause analysis is a useful and cost-effective mechanism by which to improve a variety of "adverse events and conditions." These range from JCAHO-defined "sentinel events" to excessive waiting times for appointments to parking problems.... A root cause analysis, or more aptly, a contributory factor analysis, constitutes a process or set of processes by which to further your continuous improvement endeavors.
