#3. Is this root cause analysis just another make-work, JCAHO requirement?No. Root cause analysis is an accepted quality tool in nearly every industry with the exception of health care. The quality management literature in transportation, energy, manufacturing, software design, etc., has advocated (and often required) root cause analysis for over twenty years; the use of root cause analysis is part of ISO-9000 certification. Root cause analysis has been found to be both effective and cost-effective in an extraordinarily broad range of applications. The JCAHO is forcing the health care industry to utilize root cause analysis in response to sentinel events, and thereby start to catch up with the rest of both governmental and commercial quality management practices and standards.
