#11. Who determines whether or not your root cause analysis is adequate?

You mean besides JCAHO? We recommend that as a part of your sentinel event policy  the quality of each required root cause analysis be evaluated by your risk management committee or some body established by that committee. Your sentinel event policy should include standards by which the adequacy of a root cause analysis is to be assessed, and those standards must evolve in accordance with the JCAHO's own standards.
