What are the reporting requirements regarding sentinel events and root cause analysis for Department of Defense healthcare facilities?In September of 1998, a Department of Defense Directive was published regarding the JCAHO's sentinel event policy and how military healthcare facilities were to respond to that policy. Its essence is reflected in the following: Reference: DoDD 6025.13 "The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) continues to be the accrediting body utilized by the Department of Defense (DoD) to validate military treatment facilities. All military treatment facilities, inpatient and outpatient, are required to seek and achieve JCAHO accreditation (reference). Recently, JCAHO has instituted the sentinel event reporting process. This process identifies certain patient care events as potential markers of quality problems. The process, although somewhat cumbersome, has been fully integrated into JCAHO's accreditation requirements. DoD facilities therefore, are required to comply with the sentinel event reporting process." "DoD health care facilities have an excellent accreditation history and reflect our strong commitment to the highest quality care for our beneficiaries. I am confident that the requirements will not pose an impediment to continued success in the accreditation process." Each of the Armed Services is presumed to be responsible for establishing its own specific guidelines for compliance, within the framework articulated above.
