Sentinel Events, Peer Review and Root Cause Analysis in Healthcare OrganizationsMedical Risk Management Associates is very busy creating user friendly software which facilitates and documents a thorough, credible and cost-effective root cause analysis in response to a sentinel or other adverse event or condition in a clinical environment. It is our goal to deliver to the health care industry an application that provides an uncomplicated solution for root cause analysis, and simplify the complexities of quality improvement tools.
Risk management is becoming a critical component of every health care entity's organization and function. With the progressing evolution of the sentinel event policy articulated by the Joint Commission on Accreditation of Health Care Organizations (JCAHO), for the first time in the health care industry a formal structure is being established by which such an organization is to routinely examine sentinel events and/or other adverse events. This structure is root cause analysis. Root cause analysis has a long and respected history in fields outside of medicine, including such industries as energy, manufacturing, computer programming, transportation and accident investigation. One of our goals is to facilitate any given health care organization's development of root cause analysis processes which fit that organization's individualized needs in an effective (by JCAHO and other regulatory standards) and cost-effective manner, with minimal training needs and resource expenditure. We therefore offer free resources in this arena on our website, consultative services, and are developing a software product, Root Cause Analyst®, which is designed to lead a root cause analysis team (novice or experienced) through an entire root cause analysis, from problem statement to solution assessment and final report.
